By accepting (the acceptor) the terms and conditions of participating in class “i agree”, i (“i” refers to the Zumbini with Elleesa on behalf of National Dance and Fitness Incorporated participant) agree to the following:

It is the purpose of this agreement to exempt, waive, and releases from liability for personal injury, property damage, and wrongful death, including if caused by negligence, including the negligence, if any, of releasees. “Realeasees” include Zumbini with Elleesa on behalf of National Dance and Fitness Incorporated and its directors, agents, and employees.

For and in consideration of the acceptor’s registration with Zumbini with Elleesa on behalf of National Dance and Fitness Incorporated and being allowed to participate in Zumbini with Elleesa on behalf of National Dance and Fitness Incorporated events and activities, the acceptor, on his/her own behalf and on behalf of his/her minor children, hereby waives, releases and relinquishes any and all claims against releasees for liability and any and all causes of action against releasees, including for personal injury, property damage or wrongful death, occurring to the acceptor or the acceptor’s minor children, arising out of participation in Zumbini with Elleesa on behalf of National Dance and Fitness Incorporated’s events and activities and activities incidental thereto, wherever or however they occur. This waiver and release is binding upon the acceptor’s heirs, executors, administrators, and assigns.

The acceptor on his/her own behalf and/ or on behalf of his /her heirs, executors, administrators, and assigns agrees that if any claim for the acceptor’s or his/her minor children’s personal injury, property damage, or wrongful death is commenced against releasees, the acceptor or his/her heirs, executors, administrators, or assigns shall defend, indemnify and save harmless releasees from any and all claims or causes of action by whomever or wherever made or presented for the acceptor’s or his/her minor children’s personal injuries, property damage, or wrongful death.
